CHECKS: Checks are only accepted for established clients. There will be a service charge for any returned checks. Should your check be returned for insufficient funds, you expressly authorize your account to be electronically debited or bank drafted for the amount of the check plus any applicable fees. The use of a check is your acknowledgement and acceptance of this policy and its terms and conditions. Please include the following information on your check: full name, address, phone number, and your pet's name.
PREPAYMENT: I agree to pay 50% of the total cost at the time of admission of my pet and the balance at the time of discharge. I am aware that an estimate is available upon my request prior to service.
ESTIMATES: I realize that the estimated fees may not include unanticipated procedures which may be necessary for the well-being of my pet.

No further services, products or prescriptions will be provided to your pet if there is a balance on the account. The signature below authorizes that you have been informed of our payment policies and agree to appropriate charges that might be assessed in the discharge invoice to maintain your pets level of preventative care.
